I also have been trying to reproduce patterns which might lead to incomplete shifts, and so far haven't been able to reliably reproduce the problem. It appears randomly. I think I'm noticing a trend when slowing down on a decline and shifting from 4th -> 3rd, but it doesn't happen all the time. More often it occurs when shifting from 2nd to 3rd.
