In extreme cases it technically could blow up. I have seen it the aftermath and could probably show you pictures of a blown up battery or two. However it isn't very common.

The scenario would involve a bad, severely discharged battery. Batteries in this condition create Hydrogen Gas. In this situation the battery could explode if you do anything to cause a spark like taking the positive side off first, or creating a short.

Again, it isn't very common. I work with batteries a lot (in generators) and I haven't had one blow up on me yet. But I have seen the results of several exploded batteries. It does happen. If I am changing a battery which is particularly old or if I have reason to believe it is severely discharged I take extra care around it.
