Thanks for the detailed write-up TigerDave. I did the DIY as you instructed and everything turned out as expected. In fact the door opener probably has a little better range now than it had running off of battery power.

Anyways, I combined your wiring instructions with the button placement suggested by b0yd07 and below are the results:
