Yes, there is a "hole" for the defouler to plug into. There are usally two bungs welded onto a RH. The one nearest the engine if for the primary and the one farther to the back is for the secondary which is the o2 sensor for the catalytic.
The drilling has to do with the defouler itself as where the O2 sensor screws into is not quite big enoug. Yes, you will fail inspection if they "sniff" test your car.
